Page 213 of manual question

Page 213 in the manual is "Form 3 - Page 2." The seventh footnote refers the reader to "Table G-1 on the last page of these forms." Anyone know where table G-1 is or if that's a typo?

That table is now located at the end of Section 3.3.3, and we will correct the note for the next release.
